SJCOE COVID19 Survey for High School Students
about REOPENING SCHOOLS & BELL SCHEDULES
Dear Parents and Guardians,
COVID-19 vaccination is one of the most important tools that we have to end the pandemic and -- along with school safety plans following the guidance of health experts -- a key factor in keeping classrooms open for in-person instruction.
The U.S. Food and Drug Administration (FDA) has authorized the emergency use of the Pfizer-BioNTech COVID-19 Vaccine in individuals 16 years of age and older.
The San Joaquin County Office of Education (SJCOE) is a state-authorized vaccination site that recently completed a series of vaccine clinics for school employees where more than 25,800 doses of the Pfizer vaccine were administered.
In partnership with San Joaquin County Public Health Services, the SJCOE has been offered the opportunity to vaccinate students (ages 16 to 18) throughout San Joaquin County.
The purpose of the following survey is to determine if there is parental interest in SJCOE providing this service to students.
This is an interest survey only. Should responses indicate there is significant interest for the SJCOE to proceed with a student vaccination clinic for 16- to 18-year-olds, you will be contacted with a registration link, consent form, and description of the online registration process. Below is the link to the survey.
